Our Real MedCare team provides transitional care by coordinating services that provide continuity of health care during movement from hospital to home or from hospital to assisted living facility.
Transitional care is a necessary stepping stone between hospitalization and home after an illness or surgery. It helps patients get back on their feet safely, often with the help of specialized nurses who have experience in helping people move out into society again successfully!
Transitional can include everything from post-op discharge planning to physical therapy visits that continue throughout your healing process.
Our health care providers will work to coordinate and manage the patient’s care for the first 30 days after they return home, and they will work with the patient, family, caregivers, and other health care providers.
